· This is an extensive review of the literature on quality and safety and the ways that hospitals have traditionally organized their information to achieve high performance in these areas. This would include the ways they have utilized a variety of strategies – including the traditional (paper) medical record. Within this context define:
i. Quality
ii. Safety

· How successful have hospitals been in this endeavor. This would include attention to the IOM (Institute of Medicine) study on medical errors, past efforts with the paper medical record (see Weed on the Patient Centered Medical Record) and advances since Weed.

· A clear statement of the problem facing hospitals in the US

· Modern information systems for hospitals – problems and prospects

II. The electronic medical record – its promise/potential and criticism
· This would be an extensive review of the idea of the EMR. There is a substantial literature on this in both the US and Canada – some special issues of Healthcare Papers (Canadian) have looked at this.
